A copy of the death certificate for Francis Ogrodnik, printed on two sheets. Across the top of the original was handwritten "26056-A-6-5 03." There are several handwritten checkmarks and stamps in various fields copied over from the original. Under "cause of death," it states "killed in action, details not known," and where a fingerprint would go it instead says "remains not recovered." The reverse of the certificate, copied onto a separate sheet of paper, was signed by the Witter's commanding officer. Much of the front of the original certificate has come through onto the copy of the reverse, suggesting thin paper for the original.
